TL;DR

In 40 patients treated with breast cryoablation, mammographic radiomics, radiology report embeddings and their fusion showed no detectable discrimination for biopsy-proven ipsilateral breast tumor recurrence. Fusion had the highest leave-one-out ROC AUC for post-cryoablation biopsy (0.85), but it fell to 0.69 under repeated 5-fold cross-validation, so the results are hypothesis-generating.

Key points

Summary

After breast cryoablation, benign treatment changes on imaging can look like residual or recurrent cancer, which makes surveillance hard. This exploratory study asked whether features from the pre-cryoablation mammogram, from the text of its radiology report, or from both together are associated with two later events: a post-cryoablation biopsy and biopsy-proven ipsilateral breast tumor recurrence. With 40 patients and 7 recurrences, none of the inputs separated patients who recurred from those who did not. The combined model was most associated with a later biopsy under leave-one-out evaluation, but the estimate was unstable under repeated cross-validation. The authors call for larger cohorts, fixed follow-up horizons and independent validation before any clinical use.

FAQ

Can mammographic radiomics or radiology report text predict recurrence after breast cryoablation?

Not in this 40-patient cohort. For biopsy-proven ipsilateral breast tumor recurrence, leave-one-out ROC AUCs were 0.42 for radiomics, 0.53 for report embeddings and 0.43 for their fusion, so no input showed detectable discrimination.

Which input was most associated with a post-cryoablation biopsy?

Fusion of radiomics and report embeddings had the highest leave-one-out ROC AUC (0.85, versus 0.72 for radiomics and 0.49 for report text), but it dropped to 0.69 ± 0.06 under repeated stratified 5-fold cross-validation, so the study does not claim that fusion is better.

How did the study avoid information leakage with so few patients?

It used one pre-cryoablation mammogram and report per patient, patient-level leave-one-out cross-validation, and fitted every learned preprocessing and feature-selection step inside the training folds, with repeated 5-fold cross-validation to check stability.
